Instructions for your first visit with Dr. Mulholland

We would like to take this time to welcome you to our practice as well as inform you of our office policies.

  • Your first appointment with Dr. Mulholland will be approximately 40 minutes.

  • Please provide a list of your current medications and any medical records and imaging you feel would be helpful during your consultation with Dr. Mulholland.

  • Please bring your current insurance cards and photo I.D.

  • We are contracted with most major insurance companies, please come prepared to pay your co-pay at the time of service.

  • Please complete the registration forms to the best of your ability and bring them with you to your appointment.

What you need to do before surgery

(Pre-Op)

Pre-operative testing needs to be done on all patients that are scheduled for surgery. Depending on what your health plan requires, you will have your labs, EKG, and chest x-ray done at Huntington Memorial Hospital or with your primary care physician.


Post Craniotomy Instructions

These instructions are provided as a guide to help you in recovery. Please follow these and any additional instructions provided to you by Dr. Mulholland or Kylie Smith, PA-C.


Post Cervical Spine Surgery

These instructions are provided as a guide to help aide you in recovery. Please follow these and any additional instructions provided to you by Dr. Mulholland or Kylie Smith, PA-C.


Post Thoracic/Lumbar Spine Surgery

These instructions are provided as a guide to help aide you in your recovery. Please follow these and any additional instructions provided to you by Dr. Mulholland or Kylie Smith, PA-C.

Disability Forms:

Our office will complete state or employer disability forms for the surgical and post-operative time period for a $25.00 completion fee. Please fill out your portion entirely before leaving the forms in our office. You may apply for state disability online via the EDD website. If you apply online please call the office and leave the receipt number and your return to work date for Heather so that we may complete the physician portion. When dropping forms off at our office, please include a pre-paid postage envelope so we can send them out without delay.

Short-term disability, long-term disability, and FMLA forms will be completed by our office for a $25.00 completion fee. Please include a pre-paid postage envelope when dropping off forms. If forms are to be faxed, please specify.

Patient FAQ

  • Dr. Mulholland has privileges at Huntington Hospital and USC Arcadia Hospital.

  • In the majority of cases, it does, but each policy is different. Please contact my office and billing staff for your specific insurance plan information.

  • This varies depending on the type of surgery. Please discuss this further with Dr. Mulholland or Kylie Smith, PA-C.

  • This varies, but usually two to twelve weeks depending on the type of surgery. Please discuss this further with Dr. Mulholland or Kylie Smith, PA-C.

  • Yes. This can range from minimal to severe. You will be prescribed medication for pain control.

  • Usually, 2 weeks following surgery. You will need to call the office at (626) 793-8194 to schedule a follow-up appointment.

  • Yes. A physical therapist will see you in the hospital during your postoperative course. We will also order outpatient physical therapy after you have recovered sufficiently. This will be determined during the postoperative recovery.
